The approved proposal is for the construction of the expansion of the existing septic tank/drainfield system C-76 as a part of the Ottertail Water Management wastewater treatment facilities. The expansion will increase capacity by 1,878 gallons per day. The level of treatment will be governed by the provisions of National Pollutant Discharge Elimination System (NPDESyState Disposal System (SDS) Permit No. MN 0052809 dated March 28, 1996. The approval is pursuant to Minn. Stat. chs. 115 and 116, as amended. The final plans and specifications and related information indicate that the project will consist of the installation of two 1,500-gallon septic tanks, four 100-lineal-foot trenches, associated piping and appurtenances. The Minnesota Pollution Control Agency (MPCA), its officers, employees and agents review, comment upon, and approve plans and specifications for the limited administrative purpose of determining whether there is reasonable assurance that the treatment system when constructed, will comply with the regulations and criteria of the MPCA. This approval shall not in any way relieve the Permittee or the engineer of responsibility, nor shall it make the MPCA responsible for the technical adequacy of the engineer's work. This approval shall not relieve the Permittee from complying with all conditions and requirements of the NPDES and SDS Permit and shall be retained by the Permittee with the Permit. The Permittee is responsible for obtaining a NPDES Storm Water Permit, separate from the above-mentioned wastewater discharge permit, for any construction project which 520 Lafayette Rd. The NPDES wastewater discharge permit and the approval of the final plans and specifications do not authorize the discharge of storm water from the construction site. Furthermore, this approval does not indicate that the plans and specifications meet the requirements of the NPDES Storm Water Permit Program. Any alterations or additions to the treatment system's approved plans and specifications must be submitted to the MPCA as a Plan and Specification Addendum and be approved by the MPCA prior to bid opening. Significant alternations or additions to the treatment system's approved plans and specifications, proposed after the award of the contract, must be submitted as a change order and approved by the MPCA. Significant change orders are defined as contract deviations which: 1. substantially alter the type of treatment process, or its efficiency, versatility, or reliability; and/or 2. alter the approved project schedule affecting the initiation of operation date. Significant change orders require prior approval, from the commissioner, before the work can be done. Verbal approval may be agreed to if the work is of an emergency nature. All change orders shall be retained by the city for review by the MPCA. Each change order shall include an execution date, a complete description of the change, and signatures from the municipality's authorized representative, the engineer, and the contractor. After completion of construction, two copies of the "as-built" plans and specifications on microfiche shall be submitted. The plan sheets must be provided at 24X reduction and mounted in a white aperture card. The specifications must be provided at 25X reduction in five-channel, white, 6x4 microfilm Jackets. Both documents must be filmed on negative silver film. Please return the completed proposed schedule within 30 days. HEINZE WILL BE RESPONSIBLE FOR ALL ENGINEERING, PERMITTING FEES, DISTRICT ADMINISTRATION AND INSTALLATION COSTS. DISTRICT FOR THIS PROJECT PRIOR TO THE DISTRICT STARTING THE THE THIS EXTENSION REQUEST MUST ALSO BE THERE IS ALSO A HE WILL HAVE TO DEPOSIT THE APPROXIMATE COST WITH THE PROCESS. THE BIG PROBLEM IS THAT IT WILL TAKE SOME TIME TO PROCESS THIS REQUEST THROUGH THE MPCA ONCE THE PLANS AND SPECS ARE PUT TOGETHER. LINE UNTIL THE EXTENSION REQUEST IS CERTIFIED BY THE MPCA. THIS MAY BE A PROBLEM AS THE CLUSTER SITE THAT NEEDS TO BE EXTENDED IS VERY LARGE NOW AND MPCA MAY NOT APPROVE OF ENLARGING IT FURTHER. IT IS TO BAD THAT HE DID NOT APPROACH THE DISTRICT MUCH EARLIER TO CHECK ON THIS REQUIREMENT. TO TAKE AT LEAST 4 T 5 MONTHS IF NOT LONGER, BY THE TIME ALL THE REQUIREMENTS ARE MET.
