Yes 7 If yes, describe the work that has started. Include a description of structures completed; any dredged or fill material already discharged (including type of material and volume in cubic yards); acres or square feet filled (if a wetland or other water body); and whether the work was done under an existing permit (if so identify the permit number, if possible). 8 Is this a transportation project sponsored by a government unit? We have transported roughly 20,000 lbs./ 8 yrd''3 of stones ranging for 5 inches to 30 inches that we excavated from a farmers roughly 20 miles away. 90% of the rocks are sitting on the beach out of the water where 10% are in the water. No 9 Will the project require any dewatering (the deliberate removal of water through the use of a pump, ditch, etc. to lower water levels to allow work to be accomplished)? No 10 Has an Environmental Assessment Worksheet (EAW) or Environmental Impact Statement (EIS) been completed for the project, or will it be required? No 11 Has the project gone through a Natural Heritage (endangered species) review? 12 Have you developed any mitigation plans for the portion(s) of the project that will impact public waters? No No 13 Describe TWO alternatives to the proposed project that were considered that would avoid or minimize impacts to public waters. One option may be "no build" or "do nothing". 14 Why did you choose to pursue the option proposed in this application over these alternatives? Doing nothing would be another option, as well as just putting rock on the beach with out building up the shore line to a 3 to 1 slope The option purposed follows the process that is demanded by the governing organizations that control shore management. f; 4Activity Detail Activity; Riprap (Natural Rock) How many different water sites will have this type of activity (i.e., the number of individual stream/river, ditch, lake, pond, pit, and/or wetland crossings or unique impact areas)? 1 Site Name: Riprap (Natural Rock) Site #1 i 1 Briefly describe the impact to the waterbodies at this particular site; 2 How many cubic yards of fill are proposed, if any? 3 If applicable, what is the size of the area to be filled? 4 Please choose units: Sand, rocks and geotextile fabric will be added to the beach front. 135 cubic yards 100 linear feet 5 What is the area of stream impact in square feet?1,050 square feet NOTICE: THIS IS NOT A PERMIT.
