Dear Dan & Shannon Schmitz:
Enclosed you will see that I am returning your Conditional Use Permit Application, 1 original drawing and
10 copies, for your proposed access on Lake Adley. I have reviewed the material you submitted and find
that for our Office process this Application, you need to also submit the following:
1. The $500.00 Conditional Use Permit Application Fee.
2. A copy of your deed (proof of ownership).
3. A map that identifies the lake and the lot lines of the entire parcel; include the location of existing
structures, the septic system, the driveway(s) and the proposed project. Please note that the map
you sent does not show the entire lot nor identify the lake.
4. Details identifying all proposed cut and fill areas (length, width & depth).
5. The setback of the proposal to the lake and lot lines.
6. However, it does not appear to be an issue, but the Impervious Surface Calculation Worksheet
you completed does not include any driveway(s). Once you identify them on your drawing, you
can calculate the area for the Worksheet.
I have included an Application Checklist and 2017 Planning Commission Schedule for your use.
