However, the town board is not interested in adopting it as a road at this time. In the future, should the town board be petitioned to take over this road, it will have to meet current county specifications, among other criteria, for the town board to consider taking it over. Applicant shall notify Bruce Olson Mn/Dot Sign Supervisor 48 hours before work is to start, so Bruce can arrange for the Adopt a Highway Sign to be removed by Mn/Dot forces. Bruce’s number is 320-760-6510. Applicant shall furnish and install a 18” X 56’ CMP Culvert + 2 safety aprons and bands as needed. The grates are not needed if the culvert is 24-inch or smaller diameter. Standard Plates for Culverts are attached. Applicant shall remove entrance right of mile point 37.99, and restore turf as stated below. The applicant is to furnish all material and labor to construct the entrance. This includes culvert and surfacing materials. The culvert must meet Mn/DOT size, material and installation specifications as provided in Mn/DOT Construction Specifications, 2000 Edition. The entrance is to be constructed as shown on the attached diagram with special attention to the following: The entrance is to be constructed so that at a point 15 feet from the shoulder line of the Trunk Highway it will be six inches lower than the shoulder line elevation. All raw slopes shall be topsoiled with a minimum of three inches of black dirt, seeded with a perennial grass seed, and maintained until a full covering of turf is established. (Turf must be fully established before deposit can be released.) SCANNED The entl-ance is to be completed through the seeded stage by July 15, 2011. Failure to comply with the Provisions shall be cause for forfeiture of the deposit. A $1000.00 deposit, in the form of a Cashier’s Check made out to the COMMISSIONER - DEPARTMENT OF TRANSPORTATION, has been made to assure prompt and satisfactory completion of the entrance. The deposit will be returned to applicant when, in our opinion, the project is satisfactorily completed. The deposit is to be used by the State for partial payment of State Forces, if we are required to complete or remove the entrance in the event that the applicant does complete it promptly and satisfactorily, If the cost of the State Forces exceeds the amount of the deposit, the applicant will be billed for this difference through the Office of the Attorney General. All future maintenance of the entrance shall be the responsibility of the applicant. Any mailbox support placed on Mn/Dot right-of-way must be constructed according to the attached design. Mn/Dot will furnish a mailbox support upon request. By this means construction and maintenance plans for the portion of the trunk highway under consideration may suggest alternate or improved methods or standards of construction or reconstruction to the property owner and/or lessee. REQUIREMENTS No work under this application is to be started until application is approved and the permit issued.1. Where work on or near the traveled roadway is necessary, proper signs, channelizing devices, warning lights, and barricades must be erected to protect traffic, employees, and pedestrians. All traffic control devices and methods shall conform to the Traffic Control Devices (Ml^TCD), Minnesota Standard Sign Manual, and the appropriate provisions of Standard Specification 1710. 2. Minnesota Manual on Uniform No foreign material such as dirt, gravel, or bituminous material shall be deposited or left on the road during the construction of driveway or installation of drainage facilities. 3. Roadside must be cleaned up after work is completed.4. After driveway construction is completed the permittee shall notify the Area Maintenance Engineer or his authorized representative that the work has been completed and is ready for final Inspection and approval by the Minnesota Department of Transportation. 5. No changes or alterations in entrances may be made at any time without written permission fi'om the Minnesota Department of Transportation. 6. Driveway side slopes shall be constructed 6:1 (6* horizontal to T vertical) and shall be hand finished and seeded. 7. Driveways shall be so constructed as to slope down away fi'om the shoulder line of the trunk highway according to the most recent MNDOT Standard Plate No. 9000. Approaches, of entering roadways, shall have a grade of 0.5% away fi'om the MNDOT roadway for 50’ (25’ minimum). All other entrance approaches shall have a grade of 0.5% away from the MNDOT roadway for 25’ (15’ minimum). 8. Permission is hereby granted for the construction of the driveway as described in the above application, said driveway to be constructed in accordance with MINNESOTA RULES 8810.41 - 8810.56 and subject to the above requirements and the special provisions. Certified Check or Bond may be required to insure proper restoration of highway surfaces and to cover payment for any damage to highways or State property. Taking in about 5 acres. Of these 5 acres 3.7 acres will be disturbed for the drives. The drive is for the lots which will be residential. LOCATION AND SITE DESCRIPTION The site is west of Buchanan Lake and is mainly a sandy soil (Hubbard sand) The site is an open field with grass cover. The site lies in the Ottertail River Water shed. The nearest surface water is Buchanan Lake which is directly East of site. The location coordinates are lat. 46.4529 north Ing. -95.5699 west. All Of the project is in section 2 of T134N R39W (Ottertail township Otter Tail County, Minnesota) The site is relatively flat terrain (0-5% slope) and is a free draining sand The entire site is pervious (topsoil and grass). GENERAL CONSTRUCTION PROJECT INFORMATION We are constructing a drive. The drive will be about 2900' long with a 50' R cull-tl-sac and have 24'of gravel. The drive will stay gravel. There also could be a driveway across 2 lots. i-.'' Erosion control Temporary erosion control during construction will include silt fence. Earthen berms and temporary sediment basins. We will also incorporate A policy of leavitig a grassy buffer between the lake and the roads (this will Be implemented); All temporary sediment control will be seeded and BMPs will be used. Entrance to have stone pad to minimize vehicle Tracking. Permanent erosion control will include grasses swales and ditch blocks. Small filtratioh basins and infiltration areas. Minimal water should reach The filter strip along the lake. All home sites will retain storm water on There own land or in the grasses swales of the road. Native grasses and Deep rooted grass will be encouraged to be use in the BMPs. Site conditions proximity to water The road ends 250' from Buchanan Lake. The project does not drain or discharge into a calcareous fen, impaired water, or a karst area, and no Known wells are on the site. Residential construction During residential construction storm water BMPs will be used. • Sediment control of stockpiles (silt fence seeding ect) • Sediment control of down gradient perimeters prior to construction • Inlet protection • Sediment control for temporary or permanent drainage ditches And sediment basins. Pollution Prevention Management Measures During construction all contractors will properly manage solid waste Use of dumpsters, recycling, and proper hazardous waste disposal Is required. No construction vehicles washing or washing out of concrete Trucks are allowed on site. Any leaks of fuel or oils of over 5 gallons will be Reported to the agency. MINNESOTA STATUTE 115.061 Inspection Site inspections will be done by the contractor every 7 days or every .5" 24 hr rain event. The contractor will be required to show training Documents. When site Is 70% grown In with ground cover the site Will be turned over to owner for site management. Maintenance Records will also be turned over to owner at this time.
