Permit to replace the entrance to the main dining room building with a 12' x 23' vestibule

build a storage building. Build and level 15’ wide x 175’ long an access driveway on the lot.
Scrape off top soil and prep pad for slab on grade concrete slab. Replace discarded top soil
with class 5 gravel. New fill not to exceed 400 yards. Maximum cut would be approx. 2-3
feet and maximum fill would be 12 foot only in the ditch area located at front of property.
There will be a culvert installed under driveway in existing ditch. The project is located in
