Dear Mr. Benson,
I have received your application for Conditional Use Permit. I am unable to
process it at this time for the following reasons:
The application must be completed in black ink.
An estimate of volume of fill must be included in the “Explain Your Request”
section.
The application fee of $240.00 was not included.

Because your request submitted via the Water/Wetland Projects Application
was denied as requested, the project would not be allowed even if approval was
granted through the Conditional Use Permit (C.U.P.) process.
I have included a new application form if you wish to pursue this project.
